Adult Day Health Services are designed for those who want to live independently in the community but have a health condition which needs regular monitoring including: diabetes, high blood pressure, or complications caused by stroke or heart attack
Our Philosophy of Care is rooted in a deep respect for the dignity and self-worth of each individual. Our programs emphasize feelings of accomplishment, self-worth and belonging. Individualized care plans recognize the unique circumstance of each participant and their family.
"Our initial visit to The Community Family impressed us - there were dedicated, caring professional staff including skilled nurses for medical needs. Days were filled with arts and crafts, bingo, gardening, music, dancing, wholesome meals and most importantly - a homelike environment where we knew Dad would be safe and cared for as if he was family"  - Daughter of Community Family Client.

Adult Day Health Care is an affordable and high quality alternative to in-home care, assisted living or nursing home care. Participants spend the day in one of our three safe, friendly, home-like centers and return home in the afternoon. Family members receive peace of mind knowing that their loved one is safe and well cared for.
